Tourist visa (TR): single entry $40 (3-month validity, 60-day stay), multiple entry $200 (6-month validity). Medical treatment visa (MT): $40 single entry. Required documents: passport valid 6+ months, application form, 2x2 photo (light background), flight confirmation, bank statement minimum $700 per person or $1,500 per family, and yellow fever vaccination certificate. Accommodation proof required for tourist visa; hospital letter required for medical visa. All fees non-refundable. The embassy reserves the right to request additional documents.
